What is fatty liver, and can it be reversed?

Fatty liver means fat stored inside liver cells, now the most common liver condition worldwide, driven mostly by weight, insulin resistance and alcohol. It is silent for years and usually found on an ultrasound or blood test done for something else.

The encouraging part: the liver regenerates. Losing 7 to 10 percent of body weight can clear the fat and even reverse early scarring. Cutting alcohol and sugary drinks moves the needle fastest.

It deserves respect, not panic: unmanaged, a minority progress to serious disease. A follow-up plan with your doctor, weight work and repeat tests is the whole game.

See a doctor promptly if

  • Yellowing of the eyes or skin, dark urine, pale stools, or new confusion and drowsiness: seek medical care promptly
  • Vomiting blood, black tarry stools, or a swollen abdomen with leg swelling: seek emergency care
  • Liver enzymes several times the upper limit of normal, or a platelet count that is falling on repeat tests

Where you are, and what reverses it

ResultIn shortWhat it means
Fat in the liver, no inflammationReversibleSteatosis alone. Losing 5 percent of body weight clears much of the fat. Nothing else has ever matched weight loss here.
With inflammation and liver cell injuryReversible with moreAround 7 to 10 percent weight loss is where inflammation settles. This is the stage that decides the next twenty years.
Early scarring, stages F1 to F2Can improveRoughly 10 percent weight loss reverses scarring in a meaningful share of people. Still worth every bit of effort.
Advanced scarring, stage F3Can stabilizeProgression can be halted and sometimes partly reversed, but this stage needs a liver specialist, not a diet plan alone.
Cirrhosis, stage F4Not reversibleThe goal changes to protecting what is left and screening for complications. People live many years here with good care.
How it is stagedNot by ALTA normal ALT does not rule out scarring. A FibroScan, or a FIB-4 score from routine bloods, is what actually stages it.

Weight loss percentages are averages from trials. Alcohol, sleep apnea and blood sugar all move the outcome independently. Check it against the guidelines.
