What Does a High ESR Mean?

A doctor explains what the ESR test measures, why age and anemia raise it, how it differs from CRP, and which high results actually need investigating.

The short version

  • ESR measures how fast red blood cells settle in a tube in one hour, and they settle faster when inflammatory proteins are present, so a high ESR is an indirect sign of inflammation.
  • It is a slow, non-specific marker that rises and falls over weeks, and it is also raised by age, anemia, pregnancy, kidney disease and simply being female.
  • A mildly high ESR in someone who feels well usually needs a repeat rather than a scan; a very high ESR, above about 100 mm/hour, always needs an explanation.

ESR, the erythrocyte sedimentation rate, measures how far your red blood cells fall down a thin tube in one hour. When inflammatory proteins are present in the blood, red cells stack together and sink faster, so a high ESR is an indirect sign of inflammation somewhere in the body. It tells you that something may be going on. It never tells you what or where.

It is also one of the easiest tests to over-interpret, because several completely benign things raise it.

There is no single normal number#

ESR rises with age, and it is naturally higher in women. A practical rule used widely is that the upper limit is age divided by two for men, and age plus ten divided by two for women, expressed in millimeters per hour.

By that guide, an ESR of 30 in a woman of 70 is expected. The same 30 in a man of 22 deserves a second look. Your report's printed range may not account for this, which is one reason results get flagged unnecessarily.

What raises it besides inflammation#

Anemia is the big one, because fewer red cells fall faster regardless of inflammation. Kidney disease, pregnancy, obesity, high cholesterol and monoclonal proteins all raise it too. Very high white cell counts and polycythaemia, by contrast, can push it artificially low and mask real inflammation.

This is why ESR is almost always ordered alongside a full blood count and, usually, a CRP.

They are complementary rather than duplicates. A high ESR with a normal CRP often points towards a chronic or autoimmune process, or towards one of the non-inflammatory causes above.

When a high ESR matters#

Two situations stand out. The first is a very high ESR, above roughly 100 mm/hour, which is uncommon and usually reflects significant infection, active autoimmune disease, or a blood or solid cancer. That figure earns a proper search rather than a repeat.

The second is anyone over 50 with a raised ESR and new headache, tenderness over the temples, pain in the jaw when chewing, or any change in vision. That combination suggests giant cell arteritis, which can threaten sight and is treated as a same-day problem.

What usually happens next#

For a mild elevation in someone who feels well, the standard approach is a full blood count, CRP, kidney and liver tests, a urine dipstick, and a repeat ESR in four to eight weeks. Most have settled by then.

If symptoms exist, they direct everything. Aching and stiffness in the shoulders and hips in an older adult, for example, points towards polymyalgia rheumatica far more than the number itself does.

What is a normal ESR?
There is no single number, because ESR rises naturally with age. A widely used guide is age divided by two for men, and age plus ten divided by two for women, in millimeters per hour. So an ESR of 25 is unremarkable in a woman of 60 and would be worth a second look in a man of 25.
How is ESR different from CRP?
CRP is produced directly by the liver in response to inflammation, rises within hours and falls within days. ESR reflects plasma proteins indirectly, takes days to rise and weeks to fall. CRP is better for acute illness and monitoring treatment; ESR is more useful for slowly evolving conditions.
Can a high ESR be normal for me?
Yes. Anemia, kidney disease, pregnancy, obesity, older age and a raised cholesterol all lift the ESR without any active disease. This is why an isolated mildly raised ESR in a person who feels entirely well is frequently a false alarm rather than a clue.
