Why did my doctor order thyroid antibodies?

Thyroid antibodies answer the why behind an odd TSH. High TPO antibodies point to Hashimoto's, the immune system slowly dialing the thyroid down. That is the most common cause of hypothyroidism. Different antibodies confirm Graves disease when the thyroid runs hot.

In borderline cases, positive antibodies predict who will progress to real hypothyroidism, shaping whether to watch or treat.

Positive antibodies with normal function need monitoring, not panic, often just an annual TSH. The result mostly refines the plan; it rarely changes today.

See a doctor promptly if

  • Drowsiness, confusion or slow breathing in someone known to have an underactive thyroid. This needs emergency care.
  • Very low body temperature with slowed thinking and extreme tiredness
  • A resting heart rate under 50 beats per minute with dizziness or fainting
  • New neck swelling, trouble swallowing, or a hoarse voice lasting over three weeks
  • A high TSH found during pregnancy or while trying to conceive
  • New chest pain or a racing heartbeat after starting thyroid hormone replacement

What each thyroid antibody means

ResultIn shortWhat it means
TPO antibodies, negativeExpectedMost people without thyroid disease have none. A negative does not rule out thyroid disease, because the gland can fail without antibodies.
TPO antibodies, positiveHashimoto's, usuallyThe commonest finding by far. With a raised TSH it points at autoimmune hypothyroidism. With a normal TSH it means a higher chance of becoming underactive later, and a yearly TSH rather than treatment now.
Thyroglobulin antibodiesAdds little on its ownOften ordered alongside TPO and rarely changes the answer. Its real use is in people treated for thyroid cancer, where it affects how the thyroglobulin tumor marker is read.
TRAb or TSI, positiveGraves' diseaseA different antibody and a different illness: this one switches the gland on rather than damaging it. Ordered when the thyroid is overactive, not when it is underactive.
Positive antibodies, normal TSHNot a diagnosisAround one adult in ten carries thyroid antibodies with normal function. It is a risk marker. Antibody levels are not used to monitor treatment and do not need repeating.
In pregnancyChanges the planPositive TPO antibodies raise the risk of miscarriage, preterm birth and thyroid problems after delivery, so they earn closer TSH monitoring through the pregnancy and afterward.

Antibody levels are not used to track treatment and do not need repeating once known. The TSH is what gets monitored. Check it against the guidelines.
