Is a TSH of 0.2 bad?

A TSH of 0.2 mIU/L is low: below the usual 0.4 floor, which points to an overactive thyroid or, in someone on levothyroxine, a dose that is slightly too high.

The next step is free T4 and free T3 on the same sample. If both are normal it is subclinical and usually watched; if either is high, it is hyperthyroidism and treated.

See a doctor sooner for a racing heart, weight loss without trying, heat intolerance, tremor or anxiety that is new, because those are the thyroid running fast.

See a doctor promptly if

  • Drowsiness, confusion or slow breathing in someone known to have an underactive thyroid. This needs emergency care.
  • Very low body temperature with slowed thinking and extreme tiredness
  • A resting heart rate under 50 beats per minute with dizziness or fainting
  • New neck swelling, trouble swallowing, or a hoarse voice lasting over three weeks
  • A high TSH found during pregnancy or while trying to conceive
  • New chest pain or a racing heartbeat after starting thyroid hormone replacement
