Does melatonin actually work for sleep?

Melatonin works best for shifting a body clock, at low doses of 0.5 to 3 mg. Take it at the right time. That covers jet lag, shift work and natural night owls. For ordinary difficulty falling asleep it is modest at best, minutes rather than the transformation the gummy aisle implies.

More is not better; higher doses often just add grogginess and vivid dreams.

Chronic insomnia responds far better to CBT-I, the structured behavioral treatment that beats pills in trials, than to any supplement. Melatonin is a clock tool, not a sleeping pill.

See a doctor promptly if

  • Loud snoring with pauses in breathing, waking gasping, or falling asleep during the day despite time in bed. This suggests sleep apnea and needs assessment
  • Falling asleep while driving or operating machinery, or any near-miss from sleepiness
  • Acting out dreams, shouting, or physically moving during sleep
  • Insomnia with low mood, loss of interest, or any thoughts of harming yourself
  • New insomnia with weight loss, night sweats, palpitations or breathlessness lying flat
  • Sudden loss of the need for sleep while feeling energetic and full of ideas. This needs prompt medical review

What melatonin does, at what dose

ResultIn shortWhat it means
0.5 to 1 mg, 2 to 3 hours before bedWhere the evidence isMelatonin is a timing signal, not a sedative. Small and early shifts the body clock, which is the thing it actually does well.
5 to 10 mg at bedtimeThe common mistakeThe usual shop dose is five to twenty times what trials used. Higher does not work better and makes next day grogginess more likely.
Jet lag and shift workWorksThe clearest use. Taken at the target bedtime it pulls the clock toward the new time zone.
Delayed sleep phase, the 3am body clockWorksA small early dose reliably brings sleep onset forward. This is its best documented use in ordinary life.
Ordinary insomniaModest at bestAverages around seven to twelve minutes faster to sleep. Behavioral treatment for insomnia beats it clearly and lasts longer.
Children, pregnancy, or long term useAsk firstSold as a supplement in some countries and as a prescription drug in others, with little long term safety data in children.

Strength and purity vary widely where melatonin is sold as a supplement. Where it needs a prescription, the dose on the box is the one studied. Check it against the guidelines.

For light sleepers

Foam earplugs or a steady white noise machine fix more nights than most sleep aids, and cost almost nothing. Worth trying before anything stronger.
