Does birth control cause weight gain?

For the pill, patch and ring, large reviews find no consistent weight gain beyond what time alone brings; some retain a little fluid at first. The injection is the exception, linked to real gain in a subset of users.

Bodies differ, and feeling changed on a method is a legitimate reason to switch rather than quit contraception altogether.

There are many formulations and methods; one poor experience predicts little about the rest. That conversation with your prescriber usually finds a better fit within a try or two.

See a doctor promptly if

  • Sudden severe headache, weakness on one side, chest pain, breathlessness, or painful swelling in one leg while using hormonal contraception: seek emergency care
  • Severe lower abdominal pain, fever or foul discharge in the weeks after a coil is fitted
  • Bleeding that becomes very heavy, or bleeding after sex, which needs assessment rather than a change of pill

What the evidence shows, by method

ResultIn shortWhat it means
Combined pillNo consistent gainReviews of the trials do not find a consistent effect on weight. Most people gain a little over any few years of adult life, and the pill tends to get the credit.
Progestogen-only pillNo clear effectSame picture. Some people report bloating or appetite change in the first months.
Hormonal coilNo clear effectVery low circulating hormone levels. Weight change on the trials looks like the background rate.
ImplantMixedSome studies show a small average gain, with wide variation between people. Worth knowing rather than worrying about.
Depot injectionThe exceptionThis one does show real average weight gain in the studies, more so in some groups, and it is the method where the question is a fair one to raise before starting.
Fluid versus fatDifferent thingsEarly bloating in the first two or three months is usually fluid and settles. That is not the same as gaining fat, and it is the phase where most people abandon a method that would have suited them.

The trials measure averages across groups. Individual response varies, and a method that suits somebody else may not suit you. Check it against the guidelines.

If you are buying a scale

Any consistent scale works if you weigh at the same time each morning. Smart scales that log the trend automatically make the habit stick, and the trend matters more than any single day.
