What Does a High LDL Mean?
A doctor explains what a high LDL cholesterol means, why the target depends on your overall risk rather than one number, and what genuinely lowers it.

The short version
- LDL carries cholesterol from the liver into the artery wall, and a high level means more of it is available to build up there over years, which is why it raises heart attack and stroke risk.
- Optimal is generally under 100 mg/dL (2.6 mmol/L), with 130 to 159 mg/dL (3.4 to 4.1 mmol/L) considered borderline high and 190 mg/dL (4.9 mmol/L) or above suggesting a possible inherited cause.

LDL cholesterol is the particle that carries cholesterol from the liver out to the tissues, and it is the one that deposits into artery walls. A high LDL means there is more of it circulating and available to build up over years, which is why it raises the risk of heart attack and stroke. It causes no symptoms at all, which is the whole reason it gets measured.
The number alone, though, is only half the information. What matters is your LDL alongside everything else that drives risk.
The usual bands#

To convert, divide mg/dL by 38.7 to get mmol/L. An LDL of 190 mg/dL or more in an adult, especially with early heart disease in the family, raises the possibility of familial hypercholesterolaemia, an inherited condition that is common, underdiagnosed and important to identify because it affects relatives too.
Why the target is personal#
Two people can have an identical LDL of 140 mg/dL (3.6 mmol/L) and face completely different risks. A 35 year old non-smoker with normal blood pressure has a small absolute risk over the next decade. A 62 year old smoker with diabetes and high blood pressure has a large one.
This is why doctors use risk calculators that combine age, sex, blood pressure, smoking, diabetes and cholesterol rather than treating a single line on a report. The same logic explains why some people are advised to aim well below 70 mg/dL (1.8 mmol/L) while others are simply reassured.
What actually lowers it#
The changes with the best evidence are unglamorous and cumulative:
- Replacing saturated fat, from butter, ghee, fatty meat, coconut oil and palm oil, with unsaturated fats from olive oil, nuts and oily fish
- Soluble fiber, around 5 to 10 grams a day extra from oats, beans, lentils, apples and psyllium
- Losing excess weight and increasing regular activity, which help LDL modestly and other risk factors substantially
- Stopping smoking, which does not lower LDL much but sharply reduces the harm it does
Plant stanols and sterols lower LDL by a few per cent. Red yeast rice contains a natural statin compound in variable, unregulated amounts and is not a safer alternative simply because it is sold as a supplement.
When medication enters the conversation#
Statins and related medicines are considered when estimated risk is high enough that the benefit clearly outweighs the small inconvenience and risk, or when LDL is very high regardless of calculated risk. That is a decision for you and your own doctor, based on your numbers, your history and your preferences. It is not something a report can decide for you.
