Waist Measurement or BMI: Which Tells You More?
A doctor compares waist circumference and BMI, explains where each one fails, and gives the simple ratio that works across body types and ethnicities.

If you can only measure one thing, measure your waist. It predicts the outcomes people actually care about, type 2 diabetes, heart disease, fatty liver, high blood pressure, better than BMI does, because it reflects where fat sits rather than how much you weigh in total.
The most useful version of the rule needs no chart at all: keep your waist under half your height. A person 170 cm tall aims for a waist under 85 cm. That single ratio performs at least as well as the standard cut-offs and works across sexes, heights and ethnic groups.
Why location beats quantity#
Fat stored under the skin on hips and thighs is metabolically fairly quiet. Fat packed inside the abdomen, around the liver, pancreas and intestines, is not. Visceral fat releases free fatty acids straight into the portal vein feeding the liver, along with inflammatory signaling molecules, and it is closely tied to insulin resistance.
Two people can share a BMI of 27 and have completely different risk profiles: one carrying it as muscle and hip fat, the other carrying it as a hard, protruding abdomen. BMI cannot tell them apart. A tape measure can.
Where each measure fails#

BMI was designed in the nineteenth century to describe populations, not to judge individuals, and it was calibrated on European data. That last point matters: for people of South Asian and East Asian ancestry, the risk of type 2 diabetes rises at a BMI several units lower than the standard threshold, which is why many guidelines now apply an overweight cut-off of 23 rather than 25 in these groups.
What to do with the number#
If your waist is above the threshold for your group, the useful response is not despair but direction. Losing five to ten percent of body weight preferentially reduces visceral fat. It is mobilised earlier than subcutaneous fat, which is why blood pressure, triglycerides and liver enzymes often improve before the mirror does.
Resistance training and regular walking both reduce visceral fat, including in people whose overall weight does not change much. Alcohol and sugary drinks are the two dietary items most strongly associated with abdominal fat accumulation.
Track the waist monthly, not daily, and measure at the same time of day. Bloating, a large meal and the menstrual cycle can each shift the number by a couple of centimeters without anything meaningful happening.
The honest caveat#
Neither number is a verdict on health or worth. Fitness, blood pressure, glucose, lipids and sleep all carry independent information, and someone with a slightly high waist who exercises regularly and has normal metabolic markers is in a very different position from the same measurement in someone sedentary with a raised HbA1c.
